B-Life: Inside Lisbon's Longevity Ecosystem
How a practice on the Portuguese coast brings diagnostics, movement, nutrition, and community under a single philosophy of health.
During a recent trip to Portugal, we visited the B-Life flagship clinic in Cascais. We spent time with founder and CEO Guilherme Hilsdorf, walked through the clinic, and learned about the ecosystem the team is building.
Clinic Snapshot
B-Life opened in early 2025 in Cascais, on the Portuguese coast west of Lisbon. The clinic has a medical wing, an aesthetic treatments wing, a yoga and gym studio with an ocean view, and an in-clinic physiotherapy space.
A few minutes away, in the center of Cascais, a 400 square meter private gym extends the clinical work into physical assessment and supervised training.
Around the corner sits B-Life Kitchen, the third location. It is a restaurant and event space where the nutrition team runs cooking classes, hosts community events, and welcomes everybody for a delicious meal.
On-site clinical capabilities include a Longevity Check-Up with more than 25 clinical tests and 100+ biomarkers, a Full Body Scan protocol combining 14 MRI and ultrasound sequences, and B-Life Decode, the comprehensive baseline assessment that anchors each member's program.
Across the three spaces, the clinical team works as one practice. Cardiology, sports medicine, gastroenterology, radiology, nutrition, neuropsychology, psychiatry, gynecology, internal medicine. Women's and men's health share the same insights, the same diagnostic stack, and a single plan for each member.
Philosophy
B-Life is built on a conviction: vitality is the slow work of attention, evidence, and care, maintained week after week. The practice in Cascais is the daily expression of this conviction, across the clinic, the gym, and B-Life Kitchen.
B-Life Care, the membership program, is built on integrated care, purpose-driven technology, and continuous follow-up that places health at the center of everyday life. This is supported by clinical expertise from its 40 doctors.
At B-Life Kitchen, a nutrition plan acquires the shape it needs to outlast the consultation. Recipes, ingredients, hands-on classes, and meals shared with members give the plan a place to live beyond written instructions.
Diagnostics and Personalization
Every member begins with B-Life Decode, the practice's baseline assessment. Decode is comprehensive by design, gathering more than 100 biomarkers across 12 health domains, body composition and metabolic profiling, cardiovascular and respiratory function, hormonal and inflammatory panels, genetic testing, and a full medical and lifestyle history in a coordinated overview.
The full body scan combines 14 MRI and ultrasound sequences for early detection across organs and soft tissue. At the gym, VO₂ max, resting metabolic rate, and gait and grip analysis are captured under clinical supervision. The neuropsychology team assesses cognitive performance and stress load. The nutrition team builds a profile from food intake, cooking habits, and metabolic markers in parallel.
Blood, imaging, genetics, biomechanics, cardiopulmonary capacity, nutrition, and psychological assessment feed one plan, drafted by the longevity physician with the rest of the team weighing in. The plan keeps moving as the member does, with each clinical sub-team contributing to a single living document.
The Road Ahead
The Cascais flagship is the proving ground for a bigger vision: rebuilding longevity medicine to reach beyond the top 1%.
Each piece of the practice is being refined in clinical use so that it can travel beyond a single clinic. This includes its diagnostic depth across specialties, personalization based on many different data sources, and a focus on catching directional health issues before symptoms arise.
The work in the flagship sets the standard. The method is built to be carried further.
